New Patients

LOOKING FOR A NEW DENTIST FOR YOU OR YOUR FAMILY?

It’s an important decision. We’re ready to answer all of your questions so that you’ll be comfortable with your choice. Because at Maddox Dentistry, we’re all about making our patients comfortable.

WHAT HAPPENS ON THE FIRST VISIT?

During your first visit, you will meet our team and tour the office to learn about our practice and the technology that we use. We take the time to learn about your oral health history and current condition, as well as any concerns you may have. We also answer questions about treatments that we provide.

WILL ANY PROCEDURES BE PERFORMED?

In most instances, your first visit will also include x-rays and a complete examination so that we can determine future treatment. A cleaning and any other required procedures will occur in a follow-up visit.

DO I NEED TO BRING ANYTHING WITH ME?

New patients will receive a medical history form in the mail prior to your first appointment. Please complete and return them at the time of your first visit. If you are currently on any medications, please bring a list of those as well. You may also download the paperwork here:

DO I NEED MY DENTAL RECORDS?

If you have dental records from another office, please arrange to have them sent to us before your first appointment. This will help us avoid duplicating x-rays or insurance submissions.

APPOINTMENTS

For an appointment, please call our office at (217) 546-4738. Your specific appointment time is reserved especially for you, so we appreciate the courtesy of a 24-hour notice if you need to change your time.

PAYMENT

We accept cash, check, Visa, MasterCard, and Discover.

For those patients without insurance, please plan to pay in full at the time of services. You will receive a 5% courtesy discount.  We also provide credit/debit card payment plans.  These are 3-6 month terms with equal payments.  The first payment is due at the time of service.  The remaining will be charged to your card on the same day each month.  No finance charges will be applied to your account during this time.

Patients with insurance should plan to pay any deductibles or estimated costs that are not covered. Please note that because these amounts are estimated, you may owe a balance after the insurance has been processed.

All patients are ultimately responsible for their own accounts.  Any account that has an unpaid balance after 60 days will receive an automatic 1.5% service charge.

INSURANCE

Maddox Dentistry accepts all insurance but is not a preferred provider for any network. Your insurance company will still pay for your treatment, but it may be less than a preferred provider would receive.

As a courtesy, we file insurance claims for our patients. To speed up the process, please make sure that all of your information (name, address, birth date, policy number, etc.) is current.

You will be responsible for paying any amount that insurance does not cover. We recommend that you obtain a pre-treatment estimate for procedures over $500. This will help you understand how much insurance will pay towards the procedures, less any deductibles or other limits.

Delta Dental Insurance members should pay at the time of service. Because we are not a member of this network, they automatically send all payments directly to you. You should receive your payment approximately 2 weeks after your claim is submitted.  For those who are State of Illinois Delta members, your plan is funded through the state.  Payments are not made until money is released, therefore it may be several months before you are reimbursed.  Please keep in mind that we do have payment plans which are listed above.

CARE CREDIT

For treatments over $1,000, you may apply for Care Credit. They provide several options for payment plans to fit your particular needs. We will be happy to assist you in applying for this service.
